Eligibility:

All full-time employees who work at least 30 hours per week are eligible for coverage the first of the month following 60 days of employment.

How to enroll:

Employees must submit the Cigna Medical Enrollment/Change form to Human Resources to enroll. This form is located under the Benefits Enrollment Form section on this page.

You have 30 days to enroll or waive coverage as a new hire. If you miss the new hire enrollment window, you must wait until the next open enrollment session to enroll in benefits unless you experience a qualifying event.

What happens if I leave Bascom Maple Farms?

In certain situations, you and your dependents may continue participating in medical and dental insurance through the Consolidated Omnibus Budget Reconciliation Act (COBRA). COBRA provides a federal guarantee for the continuation of health insurance coverage after your employment ends. Depending on your circumstances, you can maintain your medical and dental coverage at your own expense for up to 18 or 36 months. If you choose to enroll in COBRA, you will be responsible for paying the full monthly premiums.

My Telehealth Benefits

Employees enrolled in the Cigna medical plan have access to telehealth services through MDLive. You can consult with a board-certified physician via phone or video conference for convenient care regarding minor conditions and symptoms.

It is recommended that you register now to streamline the process when you need to obtain a consultation in the future.

Virtual wellness screenings through MDLive are covered at 100% as a preventive care service under both the High and Low plans.

For more information about copays, deductibles, or coinsurance that may apply, please refer to the Benefits Summary for both the High and Low plans.

My HRA

Bascom Maple Farms generously offers an HRA to assist employees with deductible-eligible expenses. The HRA applies to both plans, and you are automatically enrolled when you enroll in one of Bascom’s medical plans.

The HRA is administered by Cigna, which will automatically apply HRA funds to your claims when applicable. No action is required from you to activate this benefit.

Unused HRA funds at the end of a plan year will be forfeited.

My HSA

Eligibility:

If you are enrolled in Bascom’s Cigna Open Access Plus 5000 HSA (High Plan) and have no other first-dollar medical coverage, you may be eligible to contribute to and receive contributions to your personal Health Savings Account (HSA). First-dollar medical coverage includes Medicare, Medicaid, VA Benefits, another health insurance plan, and a full-purpose FSA.

What is a Health Savings Account (HSA)?

An HSA is a tax-advantaged personal savings account that can be used to pay for medical, dental, vision, and other qualified expenses now or later in life.

You can use the money tax-free to pay for eligible expenses for yourself and tax dependents such as:

  • Copays & Deductibles
  • Prescriptions
  • Dental Care
  • Contacts & Eyeglasses
  • Hearing aids
  • Laser Eye Surgery
  • Orthodontia
  • Chiropractic care

Unused funds roll over and accumulate year after year. There is no “use it or lose it” provision.

How do I open an HSA account?

To open an HSA account,  contact your current financial institution and ask if they offer an HSA. If they do, you can request to open an account with them.

How do I contribute to my HSA?
You may contribute via payroll deductions, or by making periodic direct contributions from your personal banking account. Please reference HSA contribution limits chart for more information on how much you can contribute in the plan year.

After opening an account:

  • Contact Human Resources to setup direct deposit with your HSA account information.
  • Elect how much you would like to contribute per pay on a pre-tax basis up to the IRS max contribution limit. Please reference the HSA Contribution Limit chart for more information.
  • Access your funds at any time to pay for qualifying medical expenses via your HSA debit card.

Bascom Maple Farms offers one vision plan through Cigna. This plan is for exam-only coverage and does not cover hardware (i.e., frames, lenses, etc…). You must be enrolled in the medical plan to take advantage of the limited vision benefit.

Eligibility:

All full-time employees who work at least thirty (30) hours per week are eligible for medical coverage on the first of the month following 60 days.

Cigna Vision is serviced by EyeMed and please be aware that the Cigna Vision network is different from the networks supporting the health/medical plan.

In-network coverage includes:

One vision and eye health evaluation including but not limited to eye health examination, dilation, refraction, and prescription for glasses. Please note that coverage may vary at participating discount retail and membership optical locations. Please contact customer service for specific coverage information.

My Voluntary Life and AD&D Benefits

Life and AD&D insurance can reduce financial stress when a family is faced with the premature death or disability of a wage earner.

Bascom Maple Farms offers their employees the option of enrolling in Voluntary Life and Accidental Death & Dismemberment benefits. The cost of this plan is based on age. Please see the plan certificates for detailed information.

Eligibility

All full-time employees who work at least thirty (30) hours per week and have completed 60 days of continuous employment to participate in Voluntary Life and AD&D.

Benefit Amount

Employees may choose up to a maximum of $300,000 in Life Insurance benefits. For amounts exceeding $150,000, evidence of insurability is required.

You can choose spouse coverage up to a maximum of $100,000. For coverage exceeding $30,000, evidence of insurability (EOI) is required.

If you elect child coverage, your child(ren) is covered until age 26.
